(If its not clear in the complaint, could you post as to which rule they had been breaking at the time + try to use images in your proof.)
Here, His in-game name would be like his roleplay name, i.e : John Smith. Your reason of complaint would be the rule that he has done wrong or what you are putting the complaint on him for, the rule he has broken. His punishment is what you would like him to receive. This all comes down to what hes done wrong, Rule breaking etc..
